- You give up the right to sue Miloshevski Martial Arts for injuries, illness (including COVID-19), or other damages.
- Photos/videos may be used for promotion without payment. If under 18, a parent/guardian must sign.

1. Acknowledgment of Risks
I understand that participation in martial arts, fitness, and related activities at Miloshevski Martial Arts, LLC (MMA) involves training at any facility, park, seminar, tournament, or event organized, sponsored, or approved by MMA, regardless of location. This includes, but is not limited to, all current and future locations used by MMA for classes, training, or events.
I acknowledge that participation may result in:
- Sprains, bruises, broken bones, concussions
- Serious injury, permanent disability, or death
- Risks from other participants, equipment, or facility conditions
I voluntarily accept all risks, known or unknown, and take full responsibility for my (or my child’s) participation.
2. Health & Fitness Declaration
- I (or my child) am in good physical condition to participate.
- I do not have any condition that would limit safe participation.
- I will stop training and inform MMA staff if I believe it is unsafe to continue.
3. Waiver & Release of Liability
- Release and forever discharge MMA, its owners, instructors, staff, volunteers, affiliates, and facility owners/lessors (“Releasees”) from any and all claims, liabilities, damages, or losses connected with participation, even if caused by negligence.
- Not sue Releasees for any injury, illness, loss, or damage.
- Indemnify and hold harmless Releasees from any claims, costs, or attorney’s fees arising from my (or my child’s) participation.
4. COVID-19 Acknowledgment
- COVID-19 is contagious and may cause serious illness or death.
- MMA has implemented reasonable precautions, but exposure is still possible.
- By participating, I accept all COVID-19–related risks for myself (or my child).
5. Media Release
- I grant MMA full rights to use my (or my child’s) photos, videos, or recordings taken during participation.
- This includes advertising, social media, promotional, educational, and security purposes.
- Use may be in any format, without time or geographic limits, and no royalties or compensation will be provided.
6. Parent/Guardian Section (if under 18)
- I am the parent/legal guardian of the participant named above.
- I consent to their participation.
- I agree to all terms in this waiver on their behalf.
- I accept responsibility for any claims arising from their participation.
7. Severability
If any part of this agreement is found invalid, the remainder shall still apply.
8. Acknowledgment
I HAVE READ AND UNDERSTOOD THIS AGREEMENT. I KNOW I AM GIVING UP LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. I SIGN THIS FREELY AND VOLUNTARILY, FOR MYSELF AND/OR THE MINOR PARTICIPANT.
